Application Process/Deadlines


The Community Partnership Application process has two major components:
1) Submission of proposal with supporting documents.
2) Agency Interview: After submission of proposal packet, each agency has the opportunity to clarify the proposal, as well as answer questions from the funding panel.  A short list of applicants will be contacted to set a time and date for the interview.*  No more than two agency representatives should attend.
*Due to the expedited process, there will be no interviews this year (FY09-10).

Proposals must be submitted by July 13, 2009
Each proposal should have the following items:

1) 8 copies of the completed application (DOUBLE-SIDED preferred if possible) – (Community Partnership Application 09-10).  The completed application includes:
  • List of Board of Directors, including their contact information.
  • The organization’s total operating budget.*
  • A list of all staff positions, along with their titles and salaries for the entire organization.*  Include all administrative staff.  Place an asterisk by any position that works in the program for which you are requesting funding.
*For statewide and national organizations, submit only the local chapter information.
2) 1 copy of  Certificate of Insurance - Provide insurance coverage information as outlined in Attachment A.  It is necessary to submit only one copy of each Certificate of Insurance. – (Attachment A)

3) 1 copy of your agency’s most recent audit and management letter. It is not necessary to submit a copy of your audit with each copy of your application. 
IMPORTANT: If your agency is NOT required to have an audit, please submit:
    • One copy of the Audit Form (Attachment B) – based on most recent year-end financial statements,
    • A sworn, notarized statement by the treasurer that all financial information is accurate, and
    • A completed Internal Control Questionnaire – (Attachment C).
Application Deadlines/Timetable for FY 09-10:
  • June 19: Release the RFP (RFP 09-10)
  • June 29: Information Session

  • Board of Commissioners approves the nonprofit recommendations.
