FEATURE: Community Resource Connections Open House November 16

Wake County’s Community Resource Connections (CRC) for Aging and Disabilities collaborative will hold an open house on Tuesday, November 16, 2010, from 1 to 4 p.m., on the first floor of the Wake County Community Service Center, 401 E. Whitaker Mill Road, Raleigh. The open house is for consumers, professionals, family members and friends who support seniors and adults with disabilities.

Wake’s CRC for Aging and Disabilities collaborative officially launched on May 1, 2010. The program is designed to help seniors and adults with disabilities have better access to information, referrals and various types of services.

Wake County Human Services received $45,675 in grant funding from the N.C. Department of Health and Human Services Division of Long Term Services and Supports, the state-level program manager, to establish and sustain the project.

While Wake County is the lead agency locally, the following organizations are active participants and have worked to establish common protocols to assist residents:

Members of the collaborative have created a process so there is ‘no wrong door’ for people to get the help they need. Consumers can call any organization in the collaborative to get information and access to the programs and services that all the agencies provide.
