At-Risk Case Management Services are provided to Medicaid eligible individuals that may have physical or other mental disabilities that prevent them from managing their illness or managing their household including their income.
 
At-Risk Case Management Services' primary focus is to reduce or alleviate abuse, self-caretaker neglect, and/or exploitation.
 
Services include evaluating the client’s situation to determine the need for initial or continuing case management services; assessing the service needs; developing and implementing a service plan to meet the identified needs of the client; assisting the client in locating and contacting providers and programs for needed services and the coordination of delivery of services to the client.
