Private Living Assistance (PLA) Medicaid is for the aged (65 and older), blind or disabled individuals or couples living in their own homes or apartments, or in the homes of relatives, friends, etc. If you meet the eligibility requirements, you will receive a Medicaid identification card, which is your insurance card. As with most insurance plans, there are co-payments for some services. Covered services include:

Inpatient hospital care
Doctor and clinic visits
Laboratory and X-ray services
Home health and hospice care
Medically necessary hospital transport
Eight prescription drugs per month (if covered by Medicare, drugs are covered by a prescription drug plan)
Eyeglasses
Outpatient hospital visits
Dental care
Durable medical equipment
