Postpartum/Newborn Home Visits

Home visits are provided to newly delivered women and their newborns who:

Received prenatal care through WCHS Prenatal Clinic
Received prenatal care through WakeMed High Risk Clinic
Delivered through private doctors' offices and receive Medicaid benefits
Delivered with no prenatal care.

Families receive:

Physical assessments
Parental education and support
Information about community resources
Interventions designed to promote family health and well-being.
