Form 1702

To request that medication be given to your child at school, please download the "Parent Request and Physician's Order Form" (Form 1702), to be completed by you and your child's physician.
The parent must bring this signed form as well as the medication with the original Prescription label to the Health Room staff or School Nurse.

For other health issues which require an Epi-pen, Nebulizer, Injections, or other specialized procedures, please contact your School Nurse as additional forms may be required.
