CMHRP services are provided by caring and knowledgeable care managers. These services are Medicaid approved for pregnant women.


Pregnant women are referred to these services by their prenatal care staff; however, you can refer yourself to the program.

You may be accepted for Care Management for High Risk Pregnancies if you:
  • Had contractions before 37 weeks (preterm labor)

  • Had a baby with low birth weight

  • Have unsafe/unstable living conditions such as experiencing homelessness, abuse or violence

  • Started coming to the doctor after 14 weeks pregnant

  • Are pregnant with more than one baby

  • Have an ongoing condition such as high blood pressure

Services include:
  • Being a part of your prenatal team

  • Answering any questions you may have

  • Help with making and keeping prenatal appointments

  • Guidance and support during your pregnancy and 2 months after the birth of your baby

  • Linking you to community resources
