"Helping families make a difference in their child's life"

What is Care Coordination for Children (CC4C)?

Do you worry about your child’s health? Or, whether your child can do the things other children their age can do like crawl, walk, or talk? Maybe you have a lot of stress in your life and you worry it may affect your child? Is your child acting out and you don’t know why?

CC4C is a free program for children from birth up to five years of age who have Medicaid health insurance. A CC4C care manager can talk with parents about their worries and their child’s needs. The CC4C care manager is a nurse or social worker who is trained to listen to and help parents find the right help for their child.
