Information to Bring
When you apply for Medicaid, you will be asked to provide items to prove you qualify. If you DO NOT have all the items, please apply anyway. You can provide information after you apply, and the case manager can help you if you need assistance.
- Proof of income
- Proof of resources (bank accounts, insurance policies, property, stock, bonds, vehicles, etc.)
- Proof of private health insurance
- Long-Term Care only – a form completed by a physician certifying the need for intermediate or skilled care in a nursing facility
