Energy Assistance Program

Funding:                     Federal grant

Eligibility Period:        July 1, 2005, through June 30, 2006

Who May Apply:

• Anyone age 60 or older
• Individuals under the age of 60 who are on a breathing apparatus, such as a nebulizer. (These individuals MUST have a letter from their doctor stating the medical condition, need and requirement for the breathing apparatus.)

Funding Limit:

• $300 per eligibility period per individual or household

Income Requirements:

• Assistance is determined by federal poverty guidelines. Individuals applying for assistance must provide proof of income for all individuals living within the household in order to determine eligibility.
• Items required at time of application are:
  • Social Security identification card
  • Photo ID
  • Copy of most recent paycheck stub
  • Record of any Work First payments received
  • Record of Social Security or Social Security Disability Funds received
  • Unemployment income received
  • Retirement funds received
